Who has access to these benefits?
All company employees and their immediate family members including spouses and children.
What are the requirements to qualify for these benefits?

To be an employee (employee’s spouse or children) of a company participating in the agreement with CIMA Hospital.
•
To have a valid I.D. or company I.D. and a government-issued photo I.D.
•
To show company I.D. and government-issued I.D. at the hospital or at the doctor’s office at the time of service or at your visit.
•
Family members should show the employee’s I.D. and the patient’s government-issued I.D.
